New Patient Information
Your first appointment at Dental Health Partners in Cedar Rapids will be even easier if you know what to expect.
We recommend allowing 2 hours for the initial visit. The first hour will include a full mouth series of x-rays and a complete oral exam. X-rays are necessary for proper diagnosis and to establish your needs. The dentist will talk with you one-on-one to explain your diagnosis and treatment options. This gives you time to get to know your dental care team and ask any questions that you may have. The next hour will be your professional cleaning with one of our highly knowledgeable hygenists.
You are welcome to save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
IMPORTANT:
*A parent or guardian must accompany all patients under 18 at the initial visit.*
Please alert the office if you have a medical condition that may be of concern prior to your visit (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, anxiety, etc.) or if you are on any medication (e.g. heart medications, aspirin, anticoagulant therapy, etc.)
Please be prepared to provide a list of your current medical prescriptions at the time of your initial appointment.
